Reporting Resources
There are few different options when filing a report. Please read the descriptions below to determine which form will best suit your needs.
Office of Student Rights and Responsibilities (OSRR)
If you are in immediate danger or require medical attention, please contact the police or emergency medical services at 911 or (209) 228-2677 (CAT-COPS
STUDENT MISCONDUCT
To report a potential violation of the Code of Student Conduct, please use the Information & Incident Report Form and select "Code of Student Conduct Violation" when asked what you are referring to our office.
HAZING
If you have any hazing concerns, there are a few ways to report them:
- Emailing directly to hazing@ucmerced.edu.
- Filling out our online report form and someone will follow up with you about the report you submitted.
- Requesting a meeting with either the Office of Student Involvement (clubsandorgs@ucmerced.edu and/or fsl@ucmerced.edu) or the Office of Student Rights and Responsibilities (osrr@ucmerced.edu).

Office for the Prevention of Harassment and Discrimination (OPHD)
Report incidents of discrimination, harassment, sexual harassment, and sexual violence (including dating/domestic violence and stalking) to the OPHD office in the following ways:
- Online: OPHD Web Form (Available at any time). Note: The online report form will link to an external site hosted by the University of California & iSight.
- Email: Send a report/complaint to ophd@ucmerced.edu (Available at any time)
